Diskette Submittal
The Lobbyist Disclosure Statement has three parts, three schedules, and the Signature Verification Form. (Instructions) You may download these sections in one or more of the formats listed below. Consult your software manual for the editor you plan to use (for example, Word or Word Perfect) on how to modify the files. If using Internet Explorer, point your cursor at the file you want. Then right click on your mouse button, choose "Save Target As...", and then select the appropriate folder into which you want the document saved. To download from Netscape, point your cursor at the file you want and right click on it, then choose "Save Link As". Select the appropriate folder into which you want the document saved. The Signature Verification Form must be downloded in the .pdf format. This document needs to be printed out via Adobe Acrobat Reader, physically signed, and returned along with the diskette containing the completed disclosure statement. To obtain a free copy of the Reader, visit the Adobe website.
